MLTC Disability Survey
The Saskatchewan First Nation Family and Community Institute (SFNFCI) has partnered with Meadow Lake Tribal Council (MLTC) to do an analysis of existing services for people with disabilities to identify where there are gaps of services between jurisdictions that affect the access to services for Indigenous people living on reserve in MLTC communities. We are encouraging anyone who lives in any MLTC 9 First Nations Communities and is:
- Is a service provider to a First Nations persons who has a disability (ies), and /or
- Is a caregiver to a First Nations person who has a disability(ies), and /or,
Is an Individuals who have a disability(ies), and /or,

All information will be anonymous. Nothing mentioned in these surveys will affect any of the services you receive from MLTC or any other service provider involved in this project.
